Based on current demographics, CCS program priorities are the following:

  3. Reach the Medically Uninsured and Underinsured
  4. Adult Basic Literacy Education
  5. Employment and Life Skills Training
  6. Transportation

2018 Client Data and Outcomes

  • CCS served a total of 185* adult clients
  • The Seniors C.A.N. program (Seniors Community Advocate Network) served 111 clients without duplication.
  • 83 Seniors (age 55+) participated in individual case management; an increase of 15%
  • 43 adults (ages 31 - 54) participated in individual case management; an increase of 65%
  • 10 young adults (ages 18 - 30) participated in individual case management; an increase of 40%
  • 135 clients resolved the need they had; 2 cases are still active. Client outcomes improved by 31%
  • CCS continues providing free monthly educational seminars, activities and case management at the Lodi Library. We truly value this partnership.
  • CCS actively engages with over 25 service providers to meet client needs

*Each client is counted one time annually even if they receive services multiple times

CCS provides Seniors Community Advocate Network (Seniors C.A.N.) for those 60 years and older. This program presents educational seminars about what kind of help is available and how to access it.

Results for seniors during 2018:

  • QMB/Passport Waiver Programs resulted in a minimum savings of $132/month; a 21% increase
  • Feeding Medina County provided an average of 44 bags a month (a 57% increase from last year) saving senior households about $57/month for food with increased protein foods whenever possible
  • Personal Care Pantry filled an average of 32 orders a month (an increase of 22%) saving households on average about $50/month for toiletries
  • Reduced out of pocket medical expenses by 24% with an average savings of $141/month
  • Assist with purchase of necessary medical supplies
  • Direct contact with pharmaceutical companies for prescription assistance and medical supplies
  • Annual insurance evaluation and enrollment to improve coverage and reduce out of pocket expenses
  • CareSource transportation to medical appointments

Adult Program Results

CCS bridged 43 adults (a 65% increase) to the following services:

  • Job Fair at Lodi Library resulted in 8 securing employment
  • Employment & life skills training where participants left with current “tools” for their job search and improved budget plans
  • Counseling – emotional, substance/addiction & caretaker support
  • Basic Needs – food, shelter, medical, home repair, heat
  • Financial Assistance – utilities, auto repair, prescriptions & hearing aids, glasses
  • Insurance & billing

2018 Funding

CCS is funded by local organizations, grants, and individual donors whose goal is to improve the quality of life for seniors and all citizens residing in the CCS service area.

It has allowed CCS to bring invaluable health, education and social welfare programs and services that truly improve the quality of life for all who participate in CCS activities.
